i get this Cloth-patch of an HJ-Feldscher 2nd type with a golden wreath.

What's does it mean?

Is it an original Piece?

I don't know This insignia..
But the aging at the back looks allright.
If you looks at the edge of the rzm etiket and the black stitch.
You see black aging there.

This patch corresponds to the first pattern HJ doctor sleeve insignia. On the 1st of August 1935 it was discontinued for doctors and authorised for HJ and DJ medics (Feldscher) but without the outer yellow cord. This patch remained in use until the 2nd of Sept 1938 when a new range of life rune patches for medical personnel was introduced.

Just a quick note on the RZM paper label: on original HJ doctor patches this should be blue. This is noted in the RZM Mitteilungsblatt, issue 10 dated the 4th of August 1934 page 8.
 
Because Garry posted a reply, I looked again.
This time at my laptop, not on my smartphone..
Now I see that is is no black aging, it is the stich itself. Sorry.
